Geriatric Rehabilitation

Geriatric Rehabilitation can be rather complicated due to several factors. Age, agility, living conditions and even mental health all play a role in this. Geriatric care management integrates health care and psychological care with other needed services such as: housing, home care services, nutritional services, assistance with activities of daily living, socialization programs, as well as financial and legal planning. Obviously, our role is medical, not legal. However, depending on what we see in our patient may suggest that we have family or spousal input and follow up care as well.

Some patients going through rehab do very well physically, but may be starting some dementia or forgetfulness issues. We want our patients to recover 100% physically, but we also have to look at and address any aging issues.

Some aging issues we look at are: Family Support, Home Safety, Medical Needs, Cognitive Health, Mobility, Personal Hygiene, Meal Preparation and Social Interaction. We are limited in what we can do in some of these areas. However, our goal is to have a well rounded person achieving success in all these areas. 

We can help a person get better physically and doing really well with their life in that area. We work with them and educate and encourage their "other" areas as well. We've seen patients get very socially angry for the situation they are in. We know if we can bring them back physically, their social interactions will improve. We strive to provide as much as we can to help our patients get their entire life back. 

We also know enough to advise family and or spouses if additional support is needed.
